Personal information collected may include information establishing your identity, such as first and last name, address, phone number, email address, gender, height and weight, health insurance number, as well as health information, such as all or part of your medical record, a medical certificate that includes a diagnosis (or not), whether or not you consulted a doctor or other healthcare practitioner, the date of the consultation, drugs taken and prescriptions issued.
